Found on the floor in the last room of Empyrean Cloister Upper.
Pickup timer: 30 minutes.
Combine with the Dark Singularity to make the Dark Crucible:

You assemble the Dark Crucible.

Give to the Chosen of Asheron at Cragstone, Hebian-To, or Zaikhal for an interaction:

You give Chosen of Asheron Skull of Avoren Palacost.

Chosen of Asheron tells you, "This item is not yet complete, nor is it needed any longer. The Hopeslayer has been defeated. If you wish, you may still bring me an assembled Dark Crucible to gain access to the Lair of the Hopeslayer."

Chosen of Asheron gives you Skull of Avoren Palacost.
