Description of where the template should be used and any useful information on using the template.

Include all code for the template here in between the "pre" tags, this should be geared towards being able to simply copy this block and paste it into a new page. Enclose in sharp brackets any places where information must be put in and it is unclear what the information is. For example <Plural Name>. For sections that are usually unknown it is ok to prefill them with ??. However, for sections that are almost always filled in and it is obvious what should go there, leave them blank to improve the usability of the template (do not have to erase lots of stuff first).
