At the bottom several special eaters drop a jaw that can be used to obtain a character augmentation gem. Alternatively, the jaw can be turned in for 45 million xp, 2 MMD notes, and the title "Insatiable Slayer" when given to Clemenza (72.8N, 73.4W) in Westwatch. There's a three week timer for the jaw.
